Parts A and B Without the Jargon
Original Medicare is the version Washington administers itself, without a private insurer in the middle. It comes in two pieces: Part A for hospital care and Part B for medical services. Nearly every other decision you make, from supplements to Part C, builds on top of these two parts.
Many people in Perth Amboy receive their red, white and blue card in the mail and are not sure what it actually covers. This page walks through the basics with 2026 numbers, so the rest of your choices make sense.

We do not offer every plan available in your area. Any information we provide is limited to those plans we do offer in your area. Please contact Medicare.gov or 1-800-MEDICARE to get information on all of your options.
Part A: Hospital and Facility Care
Think of Part A as the facility side of the program. It chips in when you are formally admitted to a hospital, for a limited stretch in a skilled nursing facility after a qualifying admission, for hospice, and for certain care delivered at home.
Roughly ten years of payroll taxes, yours or your spouse's, usually means Part A carries no monthly charge at all. The cost shows up when you use it: in 2026 each new benefit period opens with a $1,736 bill, and very long admissions add daily charges on top.
The clock on a benefit period begins at admission and only resets after two straight months out of the hospital or nursing facility. Two separate admissions in March and October can therefore mean paying that $1,736 twice in one calendar year.
Part B: Doctors, Tests and Outpatient Care
Part B is the everyday side: office visits, outpatient procedures, blood work, X-rays and MRIs, walkers and oxygen, plus a long menu of screenings and a yearly wellness visit.
Most enrollees see $202.90 taken each month in 2026, and higher earners pay an income-related surcharge figured from an older tax filing. Once you clear $283 for the year, your usual share is one dollar in five of whatever amount Medicare approves.
If Social Security deposits already reach your account, the charge typically comes out before the money lands. Otherwise a paper bill shows up in your mailbox every quarter.
What the Federal Program Leaves Out
- Most prescriptions you pick up at a pharmacy, which is why people add a Part D plan
- Routine dental cleanings, eyeglasses and hearing aids
- Long-term custodial care in a nursing home
- A yearly limit on what you can spend out of pocket
That last point surprises people. Without additional coverage, the 20 percent share has no maximum. Filling that gap is the job of a Medigap policy or, in a different way, a Part C plan.
Getting the Sign-Up Timing Right
The first opening is seven months wide, with your birthday month sitting in the middle and three months on each side. Applying near the front of that stretch is the easiest way to avoid a gap between old coverage and new.
Job-based insurance from a company with at least 20 workers, through your own job or a spouse's, can let you put Part B off and sign up later through a special window. Without that kind of coverage, delay is expensive: a 10 percent bump for every twelve months you skipped, attached to your premium permanently.
Let the first window slip by? Every winter, the first quarter of the year offers a general sign-up season, and benefits kick in on the first of the following month.
A Local Starting Point
Some Perth Amboy residents prefer to begin with a talk at the Office on Aging, which hosts monthly Medicare presentations at the A.J. Community Center on Olive Street. Others want a one-on-one review at home by phone. Either way, understanding Parts A and B first makes the next decisions much easier.

Is Part B mandatory once I hit 65?
It depends on where your insurance comes from. Active employment coverage at a sizable company often lets you hold off, but a retiree plan or COBRA generally does not, so confirm before you skip it.
Will doctors outside Perth Amboy take my card?
Yes, nearly all of them. Any participating provider from New Brunswick to California has to honor it, with no network to check.
Where do I sign up?
Applications go through Social Security, which takes them online, over the phone or in person. We can walk you through each screen before you start.
Every dollar amount on this page follows 2026 federal rules. We rechecked the details on October 30, 2026.
